Overview:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will have experience in social work, hospital or hospice care, with a strong background in nursing and acute care. If you have a passion for case management, pediatrics, and medical terminology, we want to hear from you.
Key Duties & Responsibilities:
* Conduct needs assessments and develop community integration plans for survivors.
* Develop, coordinate, and monitor goal-oriented rehabilitation plans to promote independence and development.
* Facilitate group programs focusing on skill development related to brain injury effects and management strategies.
* Provide education and support to survivors and families in both one-on-one and group settings.
* Offer information, referrals, and support in accessing various services within CBIS or the community.
* Assist with crisis intervention and ensuring access to essential community services.
* Provide or arrange transportation for survivor participation in rehabilitation programs and appointments.
* Organize and facilitate meetings with other service providers involved in the rehabilitation process.
* Establish and maintain collaborative relationships with health system professionals and relevant agencies.
* Write proposals for third-party funders and maintain documentation such as rehab plans and program reports.
* Evaluate program effectiveness through observation and client feedback, reporting findings to the supervisor.
* Coordinate and manage projects such as Men?s Shed, She Shed, Cognitive Digital Therapy, and others as needed.
Skills:
- Experience in social work, hospital or hospice care
- Background in social work, nursing or related field
- Knowledge of acute care practices
- Proficiency in case management
- Familiarity with pediatrics
- Strong understanding of medical terminology
