The RoleThis is one of two (2) roles reporting to the VPs of Transformation. This position will report to VP Transformation Well-Being, with the second position (Senior Director Provincial Health Transformation Health Systems) reporting to VP Transformation Health Systems. Both positions will work collaboratively to advance the transformation agenda as outlined in the Health Transformation Framework. The Sr. Director's of - Provincial Health Transformation are responsible for advancing integrated acute and community service delivery across the province, acting as a clinical, planning and coordination expert, providing leadership to the provincial Strategic Health Networks (SHNs). The role has a provincial scope and mandate to design and recommend strategy and improvement plans that will drive the achievement of targeted, measurable, and sustainable clinical and operational improvements in health and health care service delivery. The SHN's are interprofessional groups with a provincial scope and mandate to help achieve targeted, measurable, and sustainable clinical and operational improvements with health outcomes and service delivery. Network members work together collaboratively to ensure there is a strategic, provincial approach to align services, initiatives, and planning across zones and provincial programs (as appropriate) to ensure consistency, coordination, quality, efficiency, and system sustainability. Strategic Health Networks (SHNs) are co-led in a dyad leadership model by a SHN Director and Medical Lead. The Sr. Director - Provincial Health Transformation Health Systems will have responsibility for the following in focus areas that generally align with the priorities of the VP Transformation Well-Being (ex. Family Care Teams, Health of Older Adults, Child Health, Mental Health and Addictions):" SHN Leadership: The Sr. Director - Provincial Health Transformation Well-Being will ensure that, through the SHNs: o Services are strategically oriented, and outcomes are evaluatedo Services are evidence based, streamlined, consistent and drive forward innovation o Unwarranted variation in health and well-being is reduced" Transformational Change and Process Transformation: The Sr. Director - Provincial Health Transformation will be accountable for the implementation, operationalization, ongoing management and monitoring of provincially integrated strategic priorities, quality outcome metrics and programs. The role will be responsible for creating a robust and transparent measurement framework to account for the performance of the networks and to provide ongoing reports, accountability, and incentives to spread and scale successful innovative change in the system. " Visible Leadership: The role will be required to provide leadership, advice and expertise to the organization to support a provincial health system that is undertaking transformational change, large-scale integration, continuous improvement, process re-engineering and innovation to deliver on its commitment to excellence and enhanced performance to create a high performing health system.
