Demands, duties, qualifications Definition of work: Reporting directly to the Director of LTC & CSS, the Nurse Practitioner (NP) is responsible for providing geriatric primary health care and supportive care to clients and families in the community environment. The successful candidate will also be reporting indirectly to the Director, Primary Health Care & Community Health Nursing and Director, Long Term Care and Community Support Services. The NP is a member of the multidisciplinary team for the Home Dementia Care Program, a pilot project designed to provide enhanced support to individuals living in the community with moderate to late stage dementia. The successful candidate will complete community-based comprehensive geriatric assessments and support/implement strategies to reduce falls, prevent delirium and maintain optimal chronic disease management in the home. The NP will be required to select approaches of care and technology on the basis of the clients' needs and tailored to the client's social, economic and cultural resources. The successful candidate will function in the roles of practitioner, consultant, educator, leader and researcher in close affiliation with their local geriatric team. Duties of the NP within the Home Dementia Care Program include integrating advanced knowledge and experience in delivering safe, effective, quality care to geriatric clients with dementia in a community, home based setting; advocate within the health care delivery system on behalf of clients with dementia in primary care; liaise with the health care delivery system and community partners to optimize geriatric dementia care; assist in the development, promotion and implementation of best practice in geriatric primary care for dementia; interpret and communicate results of investigations to clients, families and primary physicians when appropriate; practice prescriptive authority within the scope of practice for the NP; and practice with autonomy and collaboration to perform other related duties as required. The NP will have frequent and ongoing communication with the lead physician/principal investigator, research co-investigators and other professionals/disciplines involved in the project. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.Required qualifications:? Graduation from a nurse practitioner program approved by Central Health.? Current registration as a nurse practitioner with the Association of Registered Nurses of Newfoundland and Labrador.? Current BLS for health care providers from the Heart and Stroke Foundation.? Gentle Persuasive Approach or willingness to complete next available course. ? CNA certification in Gerontology or willingness to complete next available course. ? Advanced and current knowledge in comprehensive geriatric assessments, dementia assessments and proficient knowledge in differential diagnosis of dementia, delirium and depression. ? Possess strong abilities in the areas of collaboration, clinical reasoning, professional development, interpersonal effectiveness, conflict management and teamwork.?
