FHC is a registered charity and an incorporated not-for-profit Community Health Centre (CHC), with a vision of Strong Healthy Communities. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement. We are primarily funded through the Ontario Health.</p><p><br> At FHC, we believe that health is much more than just the absence of disease. Our approach to community health encompasses the social determinants of health which includes: education, employment, isolation, food security and social supports, and utilizes a community development model to promote health, prevent disease, and strengthen community capacity. We value health equity, inclusion, community engagement, accountability & transparency, excellence and collaboration & partnerships.</p>Position Summary:<p>The dietitian role is part of an inter-professional team, providing individual assessment and counselling, group education sessions, program design and delivery as well as community outreach. Please note that the successful candidate will be providing services at FHC?s Flemingdon and Fairview sites.</p>Duties & Responsibilities:<p><strong>Individual Nutrition Counselling</strong></p><ul><li>Assess client's nutrition status by gaining an understanding of food habits or preferences, socio-economic and cultural background, psychological factors and medical profile</li><li>Develop, implement and evaluate individual nutrition care plans according to the parameters of client's health, literacy level, stages of change, culture and socio-economic factors and provide nutrition counseling to clients</li><li>Ensure quality of care by maintaining up-to-date records, monitor clients on a regular basis, and participate in case conferences, consultations, team meetings, educational sessions and quality improvement initiatives with other members of the primary health care team as required</li></ul><p></p><p><strong>Health Promotion and Educational Activities </strong></p><ul><li>Identify barriers to local food access and facilitate local community initiatives to improve access</li><li>Initiate, participate in or lead program planning, delivery and evaluation related to nutrition, exercise and healthy living</li><li>Participate in the development of multi-disciplinary health promotion programs: assessing community needs, planning, implementing and evaluating programs</li><li>Support advocacy efforts of appropriate agencies and policy makers on food issues</li><li>Develop and maintain networks with community organizations and groups in order to develop collaborative initiatives</li><li>Provide nutritional consultation and capacity building to other staff and community agencies working with clients who may need nutritional care</li><li>Maintain professional competence through participation in professional development programs, networking
