*Overview*
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have a strong background in patient care and case management, particularly with children with Autism and Special Needs. This role involves working closely with children and their families to provide support, resources, and guidance throughout their healthcare journey. The Social Worker will play a crucial role in advocating for patients' needs and ensuring they receive appropriate services.
*Responsibilities*
* Conduct assessments to identify patients' social, emotional, and environmental needs.
* Develop and implement individualized care plans in collaboration with healthcare teams.
* Provide counseling and support to patients and families dealing with medical issues.
* Facilitate communication between patients, families, and healthcare providers.
* Assist with data collection for research purposes related to pediatric care.
* Advocate for patients' rights and access to necessary services.
* Maintain accurate records of patient interactions and progress.
* Stay informed about medical terminology and special education resources relevant to pediatric patients.
* Collaborate with community resources to enhance patient care.
*Skills*
* Strong understanding of Autism and healthcare systems.
* Proficient in data collection methods and research practices.
* Knowledge of medical terminology, particularly in pediatrics.
* Experience working with children in a clinical or educational setting.
* Excellent patient care skills with the ability to empathize and communicate effectively.
* Strong case management abilities to coordinate various services for patients.
* Ability to work collaboratively within multidisciplinary teams.
* Familiarity with special education resources is a plus.
