Sanguen Health Centre exists to improve individual and community well-being by providing health and social care for people who face barriers to accessing traditional supports. This includes people living with HCV, people who use drugs, those experiencing homelessness and equity deserving communities.
Our mission is to meet the needs of those in Waterloo Region and Wellington-Dufferin-Guelph living with or at risk of Hepatitis C through the provision of education, outreach, support, and comprehensive, expert medical care.

*Management Responsibilities*
* Provide direct supervision and performance management for program staff of Primary Care Outreach and Hepatitis/HIV Clinics, as well as the clinical roles embedded within Sanguen's Social Support programming
* Liaise with Team Leads and management to support and facilitate the integration of teams, and their workflows, to better address client needs and use of resources
* Provide direct supervision of the SHHI (Supportive Housing & Health Initiative) Team Lead, including mentorship, coaching and performance management
* Develop and support staff by providing mentorship, and opportunities for training and professional development
* Liaise with Human Resources to ensure team role development and support hiring and recruitment of new staff, as needed
* Network with community collaborators to support community based initiatives
*Other Responsibilities*
* Practice from a trauma-informed, harm reduction, anti-oppression lens
* Ensure best practice in Clinical Policies and workflows
* Represent Sanguen?s values while maintaining positive community relationships
* Oversee program planning and data monitoring, identifying opportunities for growth and improvement
* Manage program budgets and service reporting
* Support MOSS program planning, coordinating and executing Sanguen?s contractual obligations
* Develop and maintain EMR workflows
* Work with Directors to sustain and grow partner relationships
* Support strategic planning efforts
*Qualifications*
* Health Professional registered with an Ontario College (e.g. CNO, OCSWSSW)
* 5+ years experience working in a clinical setting, preferably in community
* 2+ years of supervisory or management experience
* Familiarity with safer drug use practices, harm reduction principles, infectious diseases with a focus on Hepatitis C as well as other sexually transmitted and bloodborne infections
* Must be comfortable being around, discussing and working with people who use or have used drugs, and compassionate to the barriers they face
* Demonstrated experience working with people who use drugs and people who are vulnerable, living in poverty, and/or experiencing homelessness
* Experience working with other radicalized populations, Indigenous peoples, people who engage in sex work, people who have been incarcerated, people with mental health challendes and people who have experienced trauma, considered an asset
* Understand and apply anti-oppression principles daily
* De-escalation and conflict resolution skills preferred
*In addition, this position requires:*
* G-class license, and valid insurance
* Have emergency first aid (or equivalent) and BLS (Health Care Provider level preferred)
* Availability to work day and some evening hours, including some on-call responsibilities
